- Attested photocopy of teaching certificate/level/diploma.
- Applicants have to provide NOC from the applicable department.
- Attested photocopy of domicile.
- Photocopies of the Automatic National Identity Card.
- 6 x certified color photos
- Crossed Postal Order of Rs.100.00 at the desire of Director General Personnel Administration (DGPA), GHQ Rawalpindi.
